Kaioh (カイオウ) is the most powerful master of Hokuto Ryūken and the first of the Rashō of the land of Shura. The eldest biological brother of Raoh, Toki and Sayaka, he harbors a deep-seated hatred toward Hokuto Shinken and the main bloodline of Hokuto. Consumed by the demonic power of the Matōki, Kaioh is the most formidable adversary that Kenshiro faced in the second part of Hokuto no Ken.
Kaioh is the eldest of the brothers and sister of the Ryūō lineage, descended from a branch of the Hokuto Sōke. His family serves the main clan of the Hokuto Sōke as guardians at Taiseiden, the ancestral sanctuary of the lineage. At a very young age, Kaioh rejects this hereditary servitude and confides his doubts to his brother Raoh, who is also resistant to the idea of dying for a cause he rejects.
One day, a fire breaks out at Taiseiden and threatens the two heirs of the Hokuto Sōke, Hyoh and Kenshiro, who are still infants at the time. His mother soaks her clothes and rushes into the flames to protect them. She dies clutching the two children against her, faithful to the very end to her duty toward the Hokuto Sōke.
This death marks Kaioh for life. While his brothers weep, he develops a cold rage. Together with Raoh, he steals their mother's body to bury it in a lake, refusing the rites of the Hokuto Sōke. Determined to tear himself free from suffering, he vows to embrace evil and kills his beloved dog with his own hands, a gesture that horrifies Raoh. From that point on, every time an emotion threatens to resurface, Kaioh slashes his own body to punish it: by adulthood, he is covered in scars.
Kaioh becomes the student of Jukei, grand master of Hokuto Ryūken. He is trained alongside Hyoh and Han, and surpasses all his peers thanks to an extraordinary destructive will.
Hokuto Ryūken is based on striking 1109 pressure points called Keiraku Hakō, compared to the 708 Keiraku Hikō of Hokuto Shinken. Its power comes above all from Matōki (魔闘気), a demonic energy that amplifies the user's violent instincts to the point of transforming them into a Majin (魔神, demon god). Jukei himself was once overwhelmed by this aura and massacred his family before Ryuken stopped him. Kaioh, for his part, embraces this corruption without reservation.
When Jukei arranges a decisive fight between Kaioh and the frail Hyoh, he blackmails the elder: if Kaioh does not allow himself to be defeated, his brothers will be threatened. Kaioh accepts the humiliation and lets himself fall from the bridge, losing the respect of his peers in order to protect his own. This episode forever forges his resentment toward the main Hokuto bloodline.
Having become the most powerful practitioner, Kaioh takes the helm of the Land of Shura as the first of the three Rashō (羅将), alongside Han and Hyoh. Under his authority, the island descends into a regime of permanent violence, where only warriors capable of killing without hesitation deserve to survive.
To contain the demonic aura he constantly emanates, Kaioh wears a special armor, sometimes called Shura no Yoroi. It does not serve to protect him but to concentrate his Matōki, which seeps from his body, his eyes, and his mouth. The surplus energy sometimes forms a demonic silhouette floating before him, the Genma Eirei (幻魔影霊, specter of the phantom demon), a harbinger of his arrival and a symbol of terror for the inhabitants of Shura.
Fearing the potential of Hyoh and his blood ties with Kenshiro, Kaioh decides to manipulate him. Jukei had attempted to seal Hyoh's memory but had hesitated at the last moment, and Hyoh had feigned having forgotten. It is Kaioh who, through the techniques of Hokuto Ryūken, brainwashes his adoptive brother and implants a false memory in which Han and himself replace Kenshiro as brothers. Hyoh thus becomes a living instrument turned against the Hokuto Shinken lineage.
Kenshiro's arrival in the land of Shura, having come to rescue Rin, forces Kaioh to intervene personally. He abducts the young woman, reveals to her the horrors of Shura, then challenges the successor of Hokuto Shinken.
During their first duel, Kaioh overwhelms Kenshiro. He unleashes his signature techniques: the Anryū Hiha, a devastating beam of Matōki, and above all the Anryū Tenha, the ultimate technique of Hokuto Ryūken that distorts space, lifts the opponent into the air and disorients him before pulverizing his body. Even Kenshiro's Musō Tensei is neutralized by the demonic aura, which allows Kaioh to locate his opponent despite his anticipation.
Defeated and unconscious, Kenshiro is crucified on a cross shaped like the Big Dipper to be publicly executed. Shachi and the pirate Akashachi intervene to save him, and Kaioh mortally wounds Akashachi with a crossbow bolt during the escape.
Learning that Hyoh is riding toward the castle to reunite with his fiancee Sayaka, his own sister, Kaioh cold-bloodedly murders her to awaken Hyoh's Matōki. He then makes his brother believe that Kenshiro is the one who committed the murder, hoping that the two heirs of the Hokuto Sōke will destroy each other and clear the way for him.
Before Rin, Kaioh finally removes his armor and reveals a face nearly identical to that of Raoh, marked with scars. He confesses that he has always hated Raoh, and that if the truth about the Hokuto Sōke lineage were to come to light, the legend of the King of Fists would crumble.
During the final battle, Kaioh seeks to uncover the secret of Hokuto Sōke, the original art from which all branches of Hokuto descend. He traps Kenshiro in an underground chamber filled with toxic gases meant to smother his techniques, but Kenshiro manages to neutralize with a thread the vital point that allowed Kaioh to fight without breathing. The duel resumes in the open air.
Kaioh then adopts the Hokuto Sōke stance and unleashes his most violent techniques: Anryū Tenha once again, Seimyō Danretsu, and ends up charging at Kenshiro. But the latter has learned from the first fight. He counterattacks with the Kentō Shadan, which shatters Kaioh's arms. Kenshiro also reveals to him the true history of the Hokuto Sōke, read on the pillars of the Taiseiden temple, shaking his opponent's hatred for good.
With his arms broken and defeat imminent, Kaioh mentally implores his mother to give him the strength for one last blow. But Kenshiro strikes first and delivers the fatal blow. In his death throes, Kaioh understands that even with Hokuto Shinken he would have lost: his dream of domination was nothing but dust. Memories of his mother and the sacrifice she made for the heirs of Hokuto Sōke rise back to his consciousness, and in death he rediscovers a part of his humanity long buried. Before his last breath, he finds the dying Hyoh, who forgives him for his crimes; Hyoh expires in his arms. Kaioh then chooses to bury himself alongside his adoptive brother in a geyser of molten lava, the magma solidifying around them to form a natural statue. His demise puts an end to the reign of the Rashō over the land of Shura.
Kaioh masters the most destructive techniques of Hokuto Ryūken as well as the Hokuto Sōke stance, and knows how to exploit both the Keiraku Hakō and certain Hokuto Shinken techniques. His arsenal notably includes the Anryū Hiha (Matōki beam), the Anryū Tenha (ultimate technique, spatial distortion), the Seimyō Danretsu (volley of concentrated blows), the Kentō Shadan, and the Genma Eirei. He also masters the Tenryū Kokyū Hō, a breathing technique that grants access to 100% of the human body's physical capabilities.
His mount is a colossal red steed, the equivalent of Raoh's Kokuō, whose name has never been specified in canon. Kaioh also possesses an impressive regeneration ability: his wounds close in a matter of seconds, and he survived repeated strikes from the Musō Tensei. This overall power makes him the greatest adversary ever faced by Kenshiro, surpassing both Raoh and Souther in raw power. In the original animated series (1987-1988), Kaioh is voiced by Kenji Utsumi, who also lends his voice to Raoh.
